Fund Raising Goal
In this project, FijiLIFE Foundation will team up with the Fijian Government Ministry of Health and Medical Services and Diabetes Fiji Inc to initiate the “Diabetes Care On Wheels” (DCW) program with a grant of $35,500.
Typically the population does not access healthcare for diabetes unlesss they face an emergency, such as, fatique, frequent urination, or foot ulcers. It is common for people diagnosed with diabetes 10 years ago not taking medication or changing their diet.
Objective
The project seeks to prevent diabetes complications and hence reduce the cost of care in Fiji.
Approach
Main partners in the project are the Ministry of Health and Medical Services and Diabetes Fiji Inc.
The goal of the project is to strengthen diabetes health care in the rural areas of Fiji where diabetes is highly prevalent and health facilities and human resources are scarce.
Diabetes Care on Wheels proposes to fund a Diabetes Mobile Clinic vehicle staffed by diabetes trained qualified nurse practitioner, diabetes educator, and a driver to bring a blood lab to rural locations offering diabetes blood screenings and education.
The Diabetes nurse practitioner will conduct regular blood tests for diabetic patients in the mobile clinic vehicle to areas that lack public transportation to medical centres or hospitals.
The Diabetes Educator will provide education seminars and videos designed to provide comprehensive, outcome-oriented education for diabetes detection and prevention in rural settings. The primary goal of these seminars is to enhance patients’ and family members’ ability to achieve self-care in the home setting through sequential learning on changes in diet, foot care, taking medication, and fact sheet resources distribution.
Let’s work to help improve the health of rural individuals, families and communities through actions aimed at reducing the rate and incidence of diabetes among these at risk populations.
